
Intake Specialist

Schedule : Full-time, 100% on-site, five days per week
Description: The Intake Specialist serves as the first point of contact for new clients seeking home care services. This role is responsible for coordinating the intake and enrollment process, ensuring all required documentation is accurate, complete, and compliant with regulatory standards. The Intake Specialist partners with internal teams and external stakeholders to support timely service initiation while delivering a professional and client-centered experience. What You Will Do: Act as the primary contact for new client inquiries, referrals, and service requests Collect, review, and verify documentation including Medicaid eligibility, MLTC enrollment, CHHA requirements, physician orders, and identification Manage the intake process from referral through start of care, ensuring timely and compliant completion Maintain accurate client records in systems such as HHAeXchange and internal databases Communicate with clients, families, caregivers, and referral sources to provide updates and support Partner with Nursing, HR, Scheduling, and Compliance teams to ensure seamless onboarding Track and follow up on pending documentation to prevent delays in care Ensure compliance with Department of Health regulations, HIPAA standards, and agency policies Maintain organized client files for audit readiness and regulatory adherence Deliver high-quality customer service in all interactions You Will Be Successful If:
You are highly detail-oriented and ensure accuracy in documentation and compliance You can manage multiple referrals and deadlines in a fast-paced environment You communicate clearly and professionally with diverse stakeholders You proactively follow up and resolve documentation gaps You collaborate effectively across departments to support smooth onboarding You demonstrate strong problem-solving skills and adaptability You maintain professionalism and confidentiality in handling sensitive information What You Will Bring:
1-2 years of experience in home care, healthcare intake, or a related administrative role Knowledge of Medicaid, MLTC plans, CHHA, and home care eligibility requirements preferred Experience with systems such as HHAeXchange is a plus Strong organizational and time management skills Excellent verbal and written communication skills Proficiency in Microsoft Office Suite including Word, Outlook, and Excel Understanding of HIP nd regulatory compliance standards Ability to handle confidential information with discretion and integrity Bilingual skills preferred based on client population Prior experience working with Certified Home Health Agency (CHHA) intake processes, including familiarity with required documentation, eligibility, and start-of-care workflows
