
Lead Director, Population Health - Field

Role Description
Population Health drives both our clinical and economic model and is focused on delivering high-quality, cost-effective care.
As a Lead Director, Population Health, you will play a critical role in supporting our Oak Street Health patients, providers and business by designing and implementing key Population Health priorities. You will work on programs and interventions, targeted to the right patients, to improve quality and cost of care, and serve as an effective cross‑team leader working side by side with providers, care teams, operations colleagues, and corporate colleagues.
Content areas for the Director include transitions‑in‑care, pharmacy and medication adherence, HEDIS / Stars, accurate risk adjustment, preferred partner relationships, and patient engagement strategy. This role requires building relationships with clinicians across a broad spectrum, including primary care physicians, nurse practitioners, pharmacists, psychiatrists, registered nurses, licensed clinical social workers, and others.
This is a fast‑paced, high‑visibility role that ensures rapid and consistent innovation and improvement within a division, aligned with a geographic division to drive results efficiently.
This role reports directly to the Regional Vice President, with a dotted line to the central Population Health team.
Core Responsibilities
Own the execution and ongoing refinement of Population Health programs in the local division
Work closely with functional leaders in the central office to assist with strategy and program design
Collaborate with regional leaders and Care Teams to ensure effective implementation of key clinical and business priorities
Maintain accountability to key population health metrics, including quality measures, utilization measures, and financial measures
Other duties as assigned
What are we looking for?
Motivated, experienced leader with a strong background in population health
Bachelor’s degree required; graduate degree preferred (MBA, MPH)
7‑10 years of relevant experience, with 5+ years specifically in healthcare or population health, or related fields
Ability to build trust and confidence with a wide range of stakeholders
Strong leadership and influencing skills
High level of organization and detail orientation
Analytical and data‑savvy, able to develop business cases, ROI analysis, and financial analysis
Proactive communicator who connects the dots between teams and projects overall organizational objectives
Expertise in Excel, Google Apps, and project management tools
Complimented by a pleasantly persistent approach and quick learning ability in ambiguous environments
