The Hospital Liaison will build strong relationships with hospitals, case managers, and other healthcare referral sources to support admissions and census growth for our skilled nursing facility. Position Summary
The Hospital Liaison is responsible for developing and strengthening relationships with new and existing referral sources that can refer residents to our skilled nursing facility. The Hospital Liaison works closely with hospitals, healthcare professionals, and the admissions team to ensure smooth transitions for incoming residents.

Responsibilities for Hospital Liaison

Develop and maintain relationships with hospitals, case managers, and healthcare referral partners.

Identify and cultivate new referral opportunities within the healthcare community.

Review clinical charts and patient information to determine admission eligibility.

Work closely with admissions and clinical teams to coordinate resident placements.

Conduct presentations, meetings, and outreach events with healthcare professionals and community groups.

Support facility marketing initiatives and community outreach efforts.

Maintain regular communication with referral sources and facility leadership.

Qualifications & Experience Requirements for Hospital Liaison

Marketing experience required.

Admissions experience preferred.

Previous experience as a Hospital Liaison preferred.

Minimum two years of marketing experience in skilled nursing, assisted living, or healthcare.

Knowledge of Medicare, Medicaid, and private insurance processes.

Strong sales, communication, organizational, and time management skills.

Ability to review clinical charts to determine admission eligibility.

Self‑motivated with the ability to achieve business development goals.
