Job responsibilities:
Evaluate functional status and develop a treatment plan aligned with clinician goals and patient needs. Document evaluations, treatments, and outcomes in the medical record. Communicate functional changes and barriers to the care team to support care planning. Work 40 hours per week with 8-hour shifts.
