Contract length: Shift length: 12 hour shifts Start Date: ASAP Support care coordination activities across providers to promote continuity of care. Coordinate scheduling, referrals, and follow-up care with clinical and administrative teams. Document care delivered, patient response, and communications in the medical record. Work 36 hours per week with 12-hour shifts.
