The schedule for this position is Monday- Friday 2p-10p
Partner with leadership to lead a team in providing person-centered support services to individuals receiving services under 245D regulations, while fostering positive and respectful relationships with each person served.
Collaborate with leadership to develop and implement individualized service plans that reflect the needs and preferences of each individual served, while promoting their independence and community integration.
Work closely with leadership to ensure that individuals receiving services have access to quality care and services, including medical care and behavioral supports, in a safe and positive environment.
Maintain accurate documentation of services provided and comply with all applicable regulations and policies, while encouraging the team to maintain high standards of professionalism and ethics.
Coordinate transportation services as needed to individuals receiving services, promoting inclusivity and accessibility in partnership with leadership.
Participate in quality assurance and improvement activities, working with the team to continuously improve service delivery and outcomes.
Provide training and coaching to team members, fostering a positive and supportive work environment that values collaboration and continuous learning.
Assist with scheduling, including alternating on-call duties, and finding staff to fill in as needed, in partnership with leadership and the team's overall success.
Perform other duties as assigned, contributing to the overall success of the team and the individuals receiving services, while working in partnership with leadership to achieve the team's goals.
Qualifications:
Passion for providing person-centered support that values individual preferences, interests, and goals
Must have at least 2 years of full-time experience working in a Community Residential setting.
Must be mature, polite, and possess leadership abilities and demonstrate competency in the completion of the above noted job responsibilities.
Ability to assist with personal hygiene, meal preparation, home maintenance, and community outings
Physical ability to lift and move individuals and objects as required by the site
Possession of a high school diploma or equivalent
Ability to communicate effectively in English, both verbally and in writing
Possession of a valid driver's license, auto insurance, and a clean driving record
Successful completion of a DHS background clearance at no cost to you
Valid driver's license, reliable vehicle, acceptable driving record, and proof of auto insurance required
Possess current or willing to obtain First Aid training and CPR Certificate as required by the home/division.
Individual must be mature, polite, competent, and possess team playing abilities, effective listening skills and strong overall social and communication skills.
Must be proficient at High School level in speaking, comprehending, reading and writing English to fully communicate with the individuals we serve and staff members, to implement specialized medical and/or behavioral protocols and programming within the home/division, and to document required information.
Must successfully complete Thomas Allen’s Medication Administration training course and prove competency.
