Home Health Liaison

Duration: Full Time

Work Schedule:

100% FTE, Monday-Friday 8:00am-5:00pm, every third Saturday 8 am - 5 pm. You will work at UW Health Charles St. Clinic 2550 Charles St.

Additional components of compensation may include:

Evening, night, and weekend shift differential

Overtime

On-call pay

Qualifications
  • Graduate of an accredited School of Nursing/College. Required
  • BSN. Preferred
Work Experience
  • At least one year professional nurse experience within the last five years in an acute care setting. Required
  • One year of experience in home care and hospital discharge planning. Preferred
Licenses & Certifications
  • Valid RN license. Required
  • Valid State driver's license. Required
