Community Case Manager – Children & Families
Make a meaningful impact in the lives of children, adolescents, and families.

Prairie View is seeking a compassionate and motivated Community Case Manager to provide school-based, home-based, and community-based behavioral health support. This role is ideal for someone who believes in strengths-based care, values teamwork, and wants to walk alongside families as they heal, grow, and thrive.

You’ll support youth ages 2–18 (up to 22 if on Waiver ) through case management, skill-building, advocacy, and crisis intervention—helping families connect with resources, build resilience, and reach their goals.

What You’ll Do
Clinical & Community Support
Provide community-based services in schools, homes, and other community settings based on individualized treatment plans.

Build strong, trusting relationships with children, adolescents, and caregivers.

Focus on patient and family strengths to build confidence, problem‑solving skills, and resilience.

Support families through goal‑setting, crisis intervention, parenting education, and life skills coaching.

Help connect families to needed community resources and services.

Advocate for patients across schools, agencies, and community systems.

Participate in treatment team meetings and collaborate with internal and external partners.

Provide services for Prairie View’s Summer Intensive Rehabilitation Program (SIRP) as needed.

Maintain ethical, high‑quality care aligned with Prairie View standards.

Documentation & Administrative Responsibilities
Complete accurate and timely clinical documentation.

Submit progress notes within 48 hours .

Complete required monthly reporting by the 5th of each month .

Maintain organized records of time and services.

Attend supervision, team meetings, and required trainings.

Coordinate with Utilization Review to ensure services align with treatment plans.

Transportation & Community Outreach
Transport patients when required.

Maintain a valid Kansas Driver’s License and personal auto insurance.

Participate in community education and outreach efforts.

Professional Growth & Quality Improvement
Complete required CCM state training within 6 months .

Engage in continuing education, workshops, and in‑service trainings.

Utilize Relias Learning to strengthen clinical knowledge.

Participate in quality improvement initiatives and organizational goals.

Contribute to a positive, respectful, and collaborative work environment.

Qualifications
What We’re Looking For
Required Qualifications
Bachelor’s degree in behavioral health or a related field (or equivalent).

Ability to submit official academic transcripts upon request .

At least 20 years of age .

Experience or demonstrated skill in case management.

Strong communication, documentation, and interpersonal skills.

Ability to work effectively with children, adolescents, and families.

Understanding of child/adolescent development and behavioral health.

Ability to maintain boundaries, manage risk, and respond to crisis.

Kansas State licensure and required CEUs (as applicable).

Valid Kansas Driver’s License and insurable driving record.

Ability to pass background screenings (KBI, DCF, Central Registry).

Ability to complete physical restraint training and engage in active play with youth.

Basic computer and word‑processing skills.
