Position Summary
Fedcap NYC provides programs and services that change the lives of marginalized people facing barriers to economic and social wellbeing. Our Transition Planning & Transportation program (TPT) provides comprehensive, culturally competent services to individuals detained in city jails and supports their successful reentry into the community. As Reentry Mentor, you will be a core member of the Fedcap TPT team with responsibility for targeted outreach, engagement, and participant support for individuals housed in Rikers Island Department of Correction (DOC) facilities. You will be the first point of contact for individuals eligible for transition planning, helping guide them into services and maintaining consistent follow-up to promote their ongoing engagement. You will support communication, scheduling, and pre-release service connection, and may accompany individuals to community providers upon discharge.

Responsibilities
You will be a key driver of Fedcap's Transition Planning & Transportation (TPT) community partnership engagement strategy, responsible for overseeing re-entry supportive service efforts and community partner relationships across New York City. This role leads to the development, management, and expansion of strategic community partnerships and ensures that participants are matched with community resources upon release from jail. The position requires both relationship sophistication and strong operational discipline.

Strategic Community Engagement

Secure and manage a portfolio of 5-10 community resource providers across New York City.

Serve as the single accountable relationship manager for all partners across the Circle of Support (COS) network, ensuring consistent, customer-focused experience from first contact to ongoing collaboration.

Facilitate quarterly partnership committee meetings for all service providers.

Proactively engage partners through quarterly business reviews, ensuring retention, satisfaction, and opportunities for enhanced integration.

Customer Service Excellence

Ensure that all partners receive timely referrals within 48 hours of participant release from jail.

Implement a structured referral process with clear service standards and communication expectations.

Integrate Quality Assurance for survey to maintain a minimum partner satisfaction score of 90% or higher.

Monitor and resolve partner escalations and quality concerns within 48 hours of incident report.

Network Accountability & Provider Oversight

Act as the operational link between service providers and the TPT Transition Managers—ensuring timely referrals, candidate responsiveness, and post-release support.

Facilitate networking sessions with providers to review service needs.

Share quarterly outcome reports with providers including feedback summaries, performance benchmarks, and placement insights.

Performance Management & Data Reporting

Track and report on a core set of KPIs including referrals, transportation volumes, housing placements and re-entry services.

Ensure 100% accuracy of referral data in FedcapCARES and related systems, with all interactions and outcomes recorded within 48 hours of release date.

Prepare and deliver quarterly referral performance reports with actionable recommendations to service providers.

Qualifications

Associate degree in business, workforce development, public administration, or related field preferred.

Minimum 3 years of experience in workforce development, social services, or employment services, ideally in a high-volume, performance-based environment.

Pass required NYC Department of Corrections security clearance to work on Rikers Island.

Compensation
$66,300 yearly
