As a Nurse Practitioner (NP) at Center Well Senior Primary Care , you'll be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.

Provide comprehensive care to seniors following clinical standards.
Collaborate daily with your care team to ensure coordinated, high-quality care.
Manage referrals, hospital/SNF coordination, DME, and home health services.
Document care and efficiently with support from quality-based coders.
Participate in on-call rotation and support growth plans at your center.
You will report to the Associate Medical Director and report onsite and in person to the Center Well Portsmouth clinic.
Master's Degree in Nursing with board certification.
Experience with value-based care and integrated team-based models.
Experience in value-based care, primary care or Medicare Advantage settings.
Familiarity with HEDIS quality indicators and Medicare guidelines.
