All Care Hospice Provider based in Glendale; CA is currently seeking a field Clinician to visit home health and hospice patients in Anaheim area.

About the Role:

We are seeking a compassionate and dependable Visiting Home Health Aide (HHA) to join our Hospice team. This role is essential in providing comfort-focused, patient-centered care to individuals in their homes during end-of-life care. The ideal candidate is caring, patient, reliable, and committed to maintaining the dignity and comfort of our hospice patients.

Key Responsibilities:

  • Provide personal care to hospice patients, including bathing, grooming, oral care, and dressing.
  • Assist with mobility, transfers, ambulation, and positioning for comfort.
  • Support patients with light housekeeping, and maintaining a safe home environment.
  • Report changes in patient condition, behavior, or environment to the assigned Case Manager promptly.
  • Follow and implement the patient's plan of care as directed by the RN.
  • Maintain accurate, timely, and complete visit documentation in accordance with agency policy and hospice regulations.
  • Adhere to all infection control, safety practices, HIPAA, and agency protocols.
  • Participate in team meetings and in-service training as required.
Qualifications:
  • Active Home Health Aide (HHA) Certificate in the state of California (required).
  • Hospice or home health experience preferred, but not required-training provided.
  • Current CPR Certification.
  • Valid California Driver's License, reliable vehicle, and proof of auto insurance.
  • Ability to work independently in the field and follow care plans accurately.
  • Strong communication skills and a compassionate, patient-centered approach.

Schedule:
  • Day shifts
  • Weekend availability (as needed)
  • Flexible per diem caseload
