*About the Role*
Join RehabVisions as a PRN Occupational Therapist supporting patients in a Skilled Nursing Facility (SNF). You’ll deliver evidence-based, goal-oriented care that helps older adults maximize function, independence, and quality of life.
*What You’ll Do*
* Evaluate and treat SNF patients across the rehab continuum (ADLs, functional mobility, cognition, safety).
* Create and progress individualized treatment plans; set measurable goals.
* Complete accurate, timely documentation in the EMR.
* Collaborate daily with PT/SLP/Nursing/MDs and the interdisciplinary care team.
* Educate patients, families, and staff on home programs and safety.
* Present occasional in-services; support program development.
* Supervise COTAs per Minnesota regulations.
*What You’ll Bring*
* Bachelor’s, Master’s, or Doctorate in Occupational Therapy (OT).
* Minnesota OT license (or eligibility).
* SNF/geriatric experience preferred (new grads and PRN-seasoned OTs welcome).
* Strong communication, time-management, and documentation skills.
