Job Description
Heritage Health Network (HHN) is looking for a Senior Associate, Growth & Business Development to drive expansion of our Enhanced Care Management (ECM) program through strategic healthcare partnerships.

This is a high‑impact, field‑based role where you’ll work directly with PCPs, IPAs, hospitals, ED teams, and behavioral health providers to generate referrals, build long‑term partnerships, and grow membership across our health plan network.

What You’ll Do

Build and manage relationships with clinical and community partners to drive referral pipelines.

Conduct regular in‑person outreach and represent HHN in the field.

Identify and close new partnership opportunities, including executing MOUs .

Educate partners on how HHN improves P4P performance, HEDIS measures, and quality outcomes .

Own and manage a CRM pipeline , tracking referrals through enrollment.

Partner cross‑functionally with Operations and Care Management to ensure strong partner experience.

What Success Looks Like

Consistent monthly ECM enrollment growth .

Strong, active referral pipeline and partner engagement .

New partnerships launched and producing referrals .

Measurable impact on partner quality and performance metrics .

Requirements
What We’re Looking For

3+ years in healthcare business development, partnerships, or provider‑facing sales .

Experience working with physician practices, hospitals, or health systems .

Proven ability to drive growth, referrals, or revenue outcomes .

Strong consultative selling and relationship‑building skills .

Familiarity with Medi‑Cal, CalAIM/ECM, HEDIS, or value‑based care (preferred).
