
SOCIAL WORKER (LCSW/LICSW)- FULL TIME

Responsibilities
Fulfills the needs of patients, families, and community by providing Social Work services in accordance with licensure, certification, training, and regulatory requirements, and coordinates and facilitates services for patients during and after admission to the acute rehabilitation unit.
Conducts discharge planning, coordination of care, and other planning and counseling activities to move the patient through the continuum of care cost‑effectively and resource‑efficiently.
Responds effectively to new and changing work assignments.
Follows ARU patients from admission to discharge, completing a thorough Unit Program Orientation and introducing the Social Work discharge planning role.
Discharge Planning and Case Management tasks include, but are not limited to:
Facilitating coordination of continued rehabilitation and after‑discharge services such as home health therapies, outpatient rehabilitation, or continuing‑care centers (e.g., Long‑Term Care Centers and Skilled Nursing Facilities).
Supporting medical team(s) in coordinating community health care needs such as home nursing care, IV therapies, wound care, outpatient hemodialysis, etc.
Coordinating/ordering durable medical equipment.
Assisting patients and families in locating community and insurance‑related benefits and resources.
Scheduling and facilitating family meetings as needed.
Tracking medical insurance benefits and authorizations for inpatient rehabilitation stays and seeking additional authorization when needed.
Participating in three weekly interdisciplinary team meetings and collaborating with medical and therapy teams to support the discharge planning process, advocating for patient/family needs and preferences.
Providing bedside coping and adjustment support to patients and families.
Keeping patients and families updated on discharge planning tasks and progress throughout the rehabilitation course.
The position also offers opportunity to expand the unit’s psychosocial support through the re‑introduction of coping skills/adjustment groups and additional participation in Stroke and Amputee groups.

Qualifications
Bachelor’s degree in social work.
Master’s degree in social work preferred.
Three years’ experience in acute care or related healthcare field preferred.
Utilization Review/Management and/or Case Management certification preferred.
Previous experience in case management and/or discharge planning strongly preferred.
Skills
