Role Description
This is a full-time, on-site Social Worker position located in Port St. Lucie, FL. The Social Worker will provide psychosocial support to patients and families by conducting assessments, developing care plans, coordinating post-discharge services, and advocating for patients’ needs. The role involves collaboration with interdisciplinary teams to enhance patient outcomes while ensuring compliance with ethical practices and organizational policies. The Social Worker will also connect patients with community resources and provide counseling and crisis intervention as needed.
Qualifications
Strong interpersonal, communication, and counseling skills
Assessment and care planning expertise in social work
Experience in crisis intervention and knowledge of community resources
Case management and advocacy skills
Ability to collaborate with interdisciplinary teams and prioritize patient-centered care
Master’s degree in Social Work (MSW) from an accredited institution
Licensed Clinical Social Worker (LCSW) or license-eligible (preferred)
Experience in healthcare or hospital settings is an advantage
Proficiency in maintaining documentation and adhering to ethical and regulatory standards
