Responsibilities
Complete and document comprehensive psychosocial assessments in the electronic medical record
Identify care planning goals and develop interventions to promote best outcomes
Participate in Interdisciplinary Team meetings, providing insight into psychosocial and emotional needs of patients
Provide education related to funeral planning and advanced directives as needed
Conduct visits with the patient and caregiver in their residential setting
Assist with bereavement support and interventions for caregivers following the death of a patient
Participate in an on‑call rotation to make after‑hours visits when needed (e.g., patient’s death, funeral, admission)
Qualifications
MSW preferred in Tennessee and Missouri, BSW required; same in Virginia
Social Work Licensure required in South Carolina; LMSW required
Social Work Licensure required in Georgia; LMSW preferred, LBSW required
One year of social work experience in a health care setting
Excellent communication and interpersonal skills
