Pay Range : $30.70 - $46.05
Make a meaningful impact every day by supporting patients and families through some of their most challenging moments. As a Clinical Social Worker, you will provide compassionate social work services, clinical interventions, and complex discharge planning for individuals with a wide range of behavioral health needs. Working closely with an interdisciplinary care team, you will help design and implement comprehensive treatment plans that promote ongoing health, stability, and recovery after discharge.
Essential Functions
Complete psychosocial assessments in alignment with departmental and professional standards to identify emotional, social, and environmental needs related to mental health conditions, illness, and life circumstances.
Partner with patients, families, and the healthcare team to deliver clinical assessments and mental health interventions.
Develop and implement psychosocial treatment plans using appropriate social work modalities, including crisis intervention, individual and family therapy, and grief/bereavement counseling.
Facilitate family meetings with physicians and care team members to review treatment plans and coordinate post‑discharge needs for optimal recovery.
Collaborate with the Psychiatric Consult team to provide resource information and behavioral health referrals post discharge.
Maintain up‑to‑date knowledge of federal, state, and local mental health systems to facilitate timely referrals and connect patients to appropriate community resources.
Consult with Care Coordination and healthcare team members regarding community‑based mental health service referral processes.
Advocate for expanded mental health resources in partnership with state LME/MCOs.
Document all social work assessments, actions, and interventions in the Electronic Medical Record and communicate essential information to care providers and payers.
Education & Experience Requirements
Master of Social Work (MSW) from a CSWE‑accredited institution.
Clinical social work experience required.
Minimum of 3 years of experience in a mental health setting with diagnosis and treatment responsibilities.
Licensure, Certification & Registration
Full Clinical licensure in North Carolina required.
Skills & Qualifications
Proficiency in the use of DSM‑5 criteria.
Strong verbal and written communication skills.
Solid computer proficiency.
Ability to work independently and collaboratively.
Ability to build positive working relationships and support conflict reduction.
Flexibility and adaptability in a rapidly evolving healthcare environment.
Knowledge of mental health systems and reforms.
Experience in a healthcare setting preferred.
