Support Worker
Starting salary £12.71ph* rising to £12.91 after successful completion of probation and to £13.10 after a further 12 months in post. *£12.71 per hour is a supplement rate, which is regularly reviewed. Standard hourly rate is £12.21. Please be aware that this rate can be withdrawn any time.

The Green Hammerton service assists clients 24 hours a day, 7 days a week. Shifts include days, nights and sleep overs. We require enthusiastic, caring individuals to take our clients shopping, gardening and to see our therapeutic ponies, and to engage in music or the sensory room. This reward‑seeking role will be based in Greenhammerton, York (YO26).

Responsibilities
Main Duties of the Job

Prepare person‑centred support plans with clients that reflect their views and aspirations, reviewing and amending them as necessary and considering the client’s changing needs and wishes.

Support clients to fulfil their aspirations, helping them live as independently as possible, through accompanying them on social visits and events, supporting their interests and hobbies, promoting choice, arranging activities and outings, and developing their skills and knowledge.

Communicate in an open and transparent way with clients and other team members, acting with integrity and keeping promises.

Provide personal care to clients in a way that respects their choices and dignity, including assistance with washing and toileting where required.

Keep accurate records, including daily notes, with full client involvement.

Detailed Responsibilities

Provide an appropriate level of care and support to clients according to their care plan, including personal care.

Maintain accurate written and numerical records.

Perform a wide range of personal care and domestic duties specific to each service.

Carry out the physical requirements of the role, including but not limited to climbing stairs, using hoists, pushing wheelchairs, moving people and working at night. Adjustments will be considered in line with the Equality Act where these criteria cannot be met due to a disability.

Use computer packages such as Word, Outlook and the Internet, and undertake relevant training and qualification in care if required.

Work cooperatively as a member of a team.

Liaise and communicate effectively with carers, families, inspectors and other external stakeholders.

UK driving licence desirable.

Person Specification
Experience

Previous experience working in care (employment or carer at home).

Qualifications

Care Certificate, or willing to undertake the Care Certificate.

Current driver licence (desirable).
