Clinical Treatment Team Social Worker

Location
Central State Hospital, Petersburg, VA

Responsibilities
Provide comprehensive, person-centered discharge planning, treatment coordination, and clinical social work services for individuals served, including assessing and identifying discharge needs, collaborating with patients, families, CSBs and community partners to develop and implement effective discharge and aftercare plans, ensuring timely activation of benefits and referrals, and maintaining thorough, recovery-oriented documentation in accordance with DBHDS and hospital protocols; participate in interdisciplinary treatment planning; deliver therapeutic interventions such as PSR groups, supportive counseling, and education; complete initial and annual clinical assessments; contribute to ongoing evaluation of patient progress and discharge readiness; engage in continuous professional development; uphold the mission, values, and standards of the Social Work Department with compassion, respect, and legal compliance.

Knowledge, Skills, and Abilities

Knowledge of principles, theories, practices, services, and techniques of social work, preferably with the forensic and/or civilly committed mentally ill population.

Knowledge and demonstrated ability to provide case management and coordinate resources for patients.

Knowledge and demonstrated ability of advanced assessment skills.

Ability and skills in providing individual and group educational, supportive, and insight counseling as well as family therapy.

Ability to interview and gather pertinent information from patients and family.

Ability to individually and in conjunction with interdisciplinary teams, develop goals and implement treatment plans according to patient needs and problems.

Knowledge, skills and ability to communicate effectively orally and in writing.

Knowledge of NASW Code of Ethics.

Ability to perform duties at a fast pace with frequent change.

Ability to interact with other professionals as a contributing team member.

Minimum Qualifications

Master’s degree in Social Work from an accredited institution.

Demonstrated professional experience in social work practice with an emphasis on case management and discharge planning.

Working knowledge and applied experience in clinical intake and assessment, treatment planning, and the provision of individual, group, and family therapeutic interventions.

Valid driver’s license and ability to safely operate a state vehicle to transport individuals served, as needed.

Preferred

Knowledge of forensic mental health is desirable.

Prior inpatient social work experience is preferred.
