Teague Nursing and Rehabilitation in partnership with Century Rehabilitation, is seeking a reliable *Certified Medication Aide *to set up, administer and document medications and perform various resident care activities and services necessary in caring for the personal needs and comfort of our residents.

_*Job details/Responsibilities:*_
* Prepares, administers, and documents medications following accepted practices and regulations under the direction and supervision of a Licensed Nurse.
* Assists residents with activities of daily living; including bathing, dressing, grooming, toileting, changing of bed linens, and positioning in and out of bed, chair, etc. Assists residents in eating where needed, and records food intake.
* Acts appropriately under the direction of a Licensed Nurse and acts as an active member of the inter-disciplinary team.
* Demonstrates ability to adjust to changes in unit/shift assignments, to meet resident and facility needs.
* Reads and follows daily care plans; performs assigned restorative and rehabilitative procedures; monitors and records vital signs; reports changes in resident condition to Charge Nurse; documents care provision on resident record/flow sheets as required and reports accidents and incidents.
* Assists in admission, transfer, and discharge of residents
_*Qualifications:*_
* High School Diploma or GED and previous Long Term Care Nursing Assistant experience preferred.
* Experience working with patients with Alzheimer's or Dementia preferred.
* Must be listed on the Texas State Nurse Aide Registry.
* Possesses a current (not expired or lapsed) Texas State Certified Medication Tech (CMT) or Certified Medication Aide (CMA) by the State and must also be a Certified Nursing Assistant in good standing with the State.
* Must be capable of performing the essential functions of the job, with or without reasonable accommodation.
* Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required.
* Other duties as assigned by the Resident Care Coordinator or the Charge Nurse.

* Lea y sigue los planes de atención diaria; realiza procedimientos reparatorios y rehabilitativos asignados; monitoriza y registra signos vitales; informa a la enfermera encargada sobre cambios en la condición del residente; documenta la provisión de cuidados en las hojas de registro/flujo del residente según sea necesario e informa sobre accidentes e incidentes.
* Ayuda en la admisión, transferencia y descarga de residentes

* Se prefiere el diploma de secundaria o GED y la experiencia previa como asistente en enfermería a largo plazo.
* Se prefiere tener experiencia en el trabajo con pacientes con Alzheimer o Demencia.
* Debe figurar en el Registro del Asistente de Enfermería Estatal de Texas.
* Pone en posesión de un Texas State Certified Medication Tech (CMT) o Certified Medicine Aide (CMA) actual (no expirado ni caducado) por el Estado y también debe ser un Asistente de Enfermería certificado con buena reputación con el estado.
* Debe ser capaz de desempeñar las funciones esenciales del trabajo, con o sin adaptación razonable.
