Job Description
Registered Nurse – Behavioral Health Home Visits (Full Time). Sign‑on bonus of $2,500. Bilingual (Spanish reading and writing) is a plus. Enjoy flexibility; you will not be caring for an entire wing of patients. Work in Waterbury and surrounding towns.

Responsibilities

Assess assigned case load of behavioral‑health care patients and families to identify physical, psychosocial, and environmental needs. Use documentation, clinical records, interdisciplinary team reports, after‑hours reports, and on‑site evaluations.

Assume primary responsibility for the caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment.

Assess patient needs and obtain data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status, and incorporate that data into the plan of care.

Make the initial evaluation and re‑evaluate the patient’s nursing needs during each visit. Schedule follow‑up visits according to patient, family, and facility needs.

Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.

Revise the plan of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility, and other team members.

Counsel, instruct, and include the patient, facility, and family in following the interdisciplinary plan of care.

Teach behavioral‑health philosophy and pain and symptom management to staff in alternate care settings such as nursing homes, hospitals, assisted‑living facilities, etc.

Initiate, supervise, and document observance of the behavioral health aide and LPN delivery of care per agency policy.

Inform supervisor and/or facility supervisor of unusual or potentially problematic nursing facility issues.

Provide appropriate support at the time of death and perform bereavement assessment.

Requirements

High School Diploma or GED

Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing

Current, unrestricted state RN license

1+ year of experience in a clinical care setting as a nurse

Able and willing to travel within the branch/office coverage area (50% of the time)

Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws

Ability to sit, stand, bend, lift, and move intermittently, with the capability to lift 50‑100 lbs

Previous home health or hospice experience preferred

Previous experience with Homecare HomeBase preferred
