About the Role

Come join our dynamic team of community-based case managers at Arbour Community Services! We are looking for full‑time, independent, hard workers who love supporting those in need, while enjoying a flexible schedule, a supportive work environment, and competitive compensation.
Our Community Support Program Case Managers work with adults 18+ in the community, living with all types of mental health needs. We provide skills training and connections to community resources to support independent living in their communities, decrease the risk of hospitalization or relapse of substance use, and improve overall quality of life.

Essential Responsibilities

Meet client weekly to provide outreach and support services
Engage client in the treatment system and support them to implement their goals
Support client to attain skills and resources needed to maintain community tenure
Coordinate Care with the providers/contacts
Provide telephone coaching and support
Complete documentation of all meetings and contacts
Attend weekly staff/department meetings and supervision
Qualifications

Bachelor’s degree preferred in Psychology, Human Services, or a related field with one (1) year of experience providing direct care
A valid Massachusetts driver’s license
Daily access to a working, registered and insured vehicle
Excellent in both written and verbal communications
Ability to independently plan and organize one’s own schedule and client services
Knowledge of Microsoft computer systems
Strong time management skills and organization skills
Bilingual applicants encouraged
Candidates must possess appropriate citizenship work documentation
Candidates must pass a CORI and drug test, as well as other pre‑employment documentation
