The role serves individuals with moderate to severe or treatment-resistant OCD, serving adults ages 18 and older on our Belmont campus.
Job Summary
Under the direction of the Nurse Manager, Clinical Coordinator & Charge Nurse, the Community Residence Counselor (CRC) provides direct patient care and participates in therapeutic milieu within the scope of defined practice. The CRC assists residents in developing daily living skills, achieving treatment goals, and maintaining a safe and supportive environment, promoting recovery, independence, and quality of life for individuals with mental health, substance use, or developmental conditions.
Provides direct patient care in accordance with current treatment plans.
Provides and supervises community activities.
Drives and transports patients as required.
Stays awake and alert during all duties.
Assists patients in keeping various appointments.
Creates clear, concise, timely documentation and shift notes.
Establishes professional relationships with patients.
Participates in patient rounds, status checks, or teams per program guidelines.
Completes assigned patient care tasks per program guidelines and daily assignment sheets.
Manages the therapeutic milieu effectively.
Provides direction and supervision to patients as needed.
Follows hospital and program policies, procedures, and protocols.
Communicates milieu or therapeutic environment information to appropriate staff.
Reports medication side effects, physical and emotional changes promptly.
Encourages patients in activities of daily living.
Assesses sleep patterns and assists patients in developing appropriate sleep cycles.
Completes mandatory annual in-service training, including CPR and de‑escalation.
Attends and participates in rounds and case conferences when appropriate.
Physical ability for participation in patient activities.
Other duties as assigned.
Qualifications
Education
High School Diploma or Equivalent required; Bachelor’s Degree in a related field preferred.
Licenses and Credentials
Medication Administration Program Certified (MAP – Massachusetts) – Massachusetts Department of Public Health required.
Experience
One-year patient care or related experience preferred; psychiatric setting 1–2 years preferred.

Basic computer skills for use of electronic and online systems (PRISM)
Familiarity with Epic preferred
