The Referral Coordinator manages the referral process for referrals originating from HMH Physician Services Division to other physician practices or to HMH Network Facilities. The role ensures all initiated referrals are completed, reported back to the referral source, and effectively reported to all stakeholders.

Responsibilities

Oversee the referral process for assigned providers, ensuring accurate data entry of demographic information, referral specialty and destination, and authorizations (if needed). Work with providers to identify urgent versus routine referrals and prioritize workflow accordingly.

Ensure all related orders are placed in the EMR to validate the referred service.

Confirm that the referred service is scheduled and notify the patient of their appointment, entering the status into Epic for stakeholder awareness.

Monitor patient attendance; if the patient cannot be seen or has not accepted the appointment, alert the referring provider.

Follow up with the patient and consulting provider to confirm the patient was seen and to obtain a status report or assessment.

Update referral status in the EMR and generate status reports as needed by stakeholders.

Participate in performance improvement activities to address referral loop closure issues, patient satisfaction, and provider satisfaction concerns.

Promote patient satisfaction through friendly, helpful service, and communicate potential issues to the manager.

Complete other assigned duties and projects in a timely manner and actively participate in required meetings.

Qualifications

High School diploma, general equivalency diploma (GED) or equivalent.

Two years of progressively responsible medical receptionist/medical office experience.

Preferred

Secretarial science education or comparable experience.

Post‑high‑school course in medical terminology and secretarial skills.
