Requirements

• Active, unrestricted Idaho medical license (in hand) or IMLC accepted
• DEA
• Board Certified
• BLS
• Proof of COVID vaccination
• NPDB report (issued within the last 30 days)
• CV in MM/YYYY format with all 30+ day gaps explained
