Care Partner Support at Home

to join the Resthaven Community Services Team in providing comprehensive coordination and individualized case management, and planning in partnership with Support at Home individuals.

Located at our

Northern Community Site

at

Elizabeth , this role is offered as a

full-time fixed term (12 months) contract

opportunity. In this role, it is expected you will demonstrate personal responsibility, effective communication, and adaptability while consistently delivering high-quality service aligned with Resthaven’s values. This includes clear and respectful communication, active listening, and providing timely updates.

Selection Criteria
Essential

Appropriately trained aged care workers with relevant experience.

Program Coordination, Team Leader, case management experience

An empathy and understanding of issues for older people, their families, and carers

Knowledge and experience of quality systems for evidence-based care coupled with effective written and verbal communication skills

A commitment to supporting older people to remain at home in the community

Demonstrated ability to work in a holistic, person centered model of care

An ability to work with individuals’ to identify their needs, develop personal goals and develop responsive packages of services

Demonstrated computer literacy and familiarity with MS Office Suite and electronic client care systems

A current SA driver’s license

Desirable
The following qualifications are highly recommended:

Diploma of Nursing

Certificate III in individual Support (Ageing) or Health Services Assistance

Certificate IV in Aged Care, Disability or Community Services

Diploma of Community Services (Case Management)

Diploma of Ageing Studies and services
