is seeking a dedicated and experienced
RN/LPN
MDS Coordinator to join our team full time, for a 40 hr position. This is an excellent opportunity for a motivated individual with a strong background in MDS coordination.
Responsibilities
Oversee and manage the MDS process to ensure accurate and timely submissions.
Coordinate interdisciplinary assessments and care plans.
Qualifications
Must have current RN or LPN licensure in the state of Massachusetts.
Proven experience with MDS coordination in a healthcare setting.
Strong organizational and communication skills.
Ability to work effectively in a team-oriented environment.
This is an onsite position only.
