Position Summary
The CLTS Waiver Program is one of Wisconsin’s Home and Community Based Services (HCBS) Medicaid Waiver programs, federally authorized under 1915(c) of the Social Security Act. HCBS waiver programs provide Medicaid funding that may be used to fund allowable support and services that promote individuals in the waiver, enabling them to access and include them in their home and community. Eligible participants include youth aged 0‑22 with developmental, severe emotional, and/or physical needs who are also functionally and financially eligible for Medicaid.

The Support and Service Coordinator (SSC) coordinates and facilitates access to all services and support—both formal and informal—needed by the child and family to meet their identified needs. The SSC uses knowledge of available programs, resources, and services to create an individualized service plan that best supports the child and family, promoting self‑determination, participation in community life, and overall well‑being.

Essential Functions

Coordinate and facilitate services with the child and family to meet developmental, emotional, and physical needs of the enrolled youth in CLTS.

Provide family‑centered, trauma‑informed, strength‑based, culturally intelligent services through monthly collateral contact and direct contact with the family at least every three months, with face‑to‑face contact with the child or youth at least every six months; one annual face‑to‑face contact must occur at the child or family’s residence.

Offer community‑focused services that promote inclusion and access for the youth and family.

Assist in establishing and maintaining all aspects of program functional and financial eligibility.

Assess and periodically reassess the child or youth’s health, safety, and functional capacity.

Identify vulnerable and high‑risk children and coordinate heightened care with other agencies when appropriate.

Provide crisis and critical incident intervention and resolution.

Act as a mandated reporter and make required reports to the Wisconsin Department of Health Services regarding the mental health, safety, or well‑being of a child or youth.

Develop and execute family‑centered service plans to meet individualized needs.

Authorize support and services provided through the CLTS Program.

Manage the coordination of benefits for services in the service plan to ensure high quality services.

Advocate and provide resources to the child, youth, and family to independently obtain access to services, regardless of funding source.

Provide transitional support for CLTS Program enrollment and disenrollment.

Complete required training, including CLTS‑specific training, shadowing, functional screen training, and mandated reporter training.

Compile and maintain required documentation, completing all paperwork in a strength‑based, youth/family‑centered manner.

Follow all SSC policies and procedures outlined by Milwaukee County Children, Youth, and Family Services.

Other Duties and Responsibilities

Attend required training and participate in staff meetings, weekly or monthly consultations, as needed.

Assist with child and family coverage for co‑workers when requested or directed.

Perform other job‑related duties necessary to carry out the responsibilities of the position.

Job Qualifications
Knowledge, Skills and Abilities

Working knowledge of youth and young adults experiencing disability needs and an understanding of traumatic life situations.

Ability to interact with escalating youth and/or parents in a calm and professional manner; follow oral and written instructions; respond appropriately in crisis situations.

Proficiency with Microsoft Word and Excel; perform basic computer functions; manage time efficiently.

Ability to multi‑task and show sensitivity toward cultural, ethnic, and disability needs.

Minimal Qualifications

Bachelor’s degree in a health or human services‑related field and at least one year of experience working with individuals with disabilities.

Complete all mandatory CLTS training and shadowing.

Working knowledge of computer applications (Windows, Microsoft Word); preferred experience with additional applications.

Ability to provide strength‑based, family‑centered, trauma‑informed, and culturally sensitive services to children and families with disability, emotional, physical, and life needs.

Knowledge and skills to work with children, young adults, and families.

Valid driver’s license, automobile, and insurance sufficient to meet agency requirements.

Meet all employee requirements, including references, criminal background check, and driver’s license check.

Work Relationship and Scope
Reports directly to the CLTS Supervisor. Maintains contact with a wide variety of individuals, including youth, family members, other program staff, collateral contacts, formal and informal family supports, funders, Milwaukee County Department of Health and Human Services Children, Youth, and Family Services Division, Milwaukee Public Schools staff and administrators, staff of youth disability‑serving agencies, and the general public.

Working Conditions
Work is performed primarily in a busy office environment serving children, young adults, and families. Some tasks are stationary at a desk using a computer; outreach takes place in the community and families’ homes across Milwaukee County neighborhoods, often requiring several hours per day of driving. Hours average 40 per week with a flexible schedule that may include evenings and weekends.

Physical Demands
Activities may include monitoring or interacting with youth, use of physical intervention in extreme situations, and driving.
