- Schedule: June 15 (7am) - 19 (7pm), 2026; 24 hour call
- Job Setting: Hospital; 268 beds; Trauma Level 1
- Types of Cases: All ages; EMR: Cerner
- Credentialing: Emergency credentialing available; completed applications due within 7 days of receipt; case logs and vaccination records required
- Board Certified/Board Eligible: Must be BC or BE in ENT
- Certifications: DEA required; COVID-19 vaccine required (medical/religious exemptions and declinations allowed); Flu vaccine required (medical/religious exemptions and declinations allowed)
- Licensure: AZ license required; minimum 1 year experience required
