Our hospice team is seeking a compassionate Nurse Case Manager to deliver high-quality, patient-centered care across the Denver area. This role focuses on managing a dedicated caseload, coordinating care across the continuum, and supporting patients and families with dignity and comfort throughout end-of-life care.

Key Responsibilities:
Manage hospice patient caseload from admission through end-of-life care
Perform initial and ongoing patient assessments
Develop and update individualized care plans based on patient needs
Collaborate with physicians to manage symptoms and ensure patient comfort
Coordinate care with an interdisciplinary team
Serve as the primary point of contact for patients and families
Maintain accurate documentation and ensure regulatory compliance
Qualifications:
Active Registered Nurse (RN) license
Minimum 1 year of hospice, home health, or related experience preferred
Strong clinical assessment and care planning skills
Excellent communication and collaboration abilities
Compassionate, patient-centered approach to care
