Overview
This Senior Case Manager role requires advanced practice skills and judgment. Candidates must have at least two years of experience in a specialized practice area of social work, with one year at a GS‑11 grade level.
Responsibilities
Verify Veterans' eligibility for VA medical care and program participation.
Ensure homeless Veterans are considered primary participants for program acceptance.
Conduct psychosocial assessments to determine needs of Veterans and family members.
Participate actively in treatment planning with multidisciplinary teams.
Maintain compliance with VA policies and documentation requirements.
Qualifications
United States citizenship; non‑citizens may be considered only when qualified citizens cannot be recruited.
Master’s degree in social work from a CSWE‑accredited school.
State licensure or certification to practice social work at the master’s level.
Proficiency in spoken and written English as required by VA Handbook 5005 – Part II – Chapter 3.
Advanced practice clinical licensure and at least two years of experience post‑licensure.
Demonstrated knowledge, skills, and abilities in specialized interventions, treatment modalities, and advanced case management.
Ability to incorporate complex causation into differential diagnosis and treatment.
Experience in developing outcome evaluations and writing policies or practice guidelines.
Valid driver’s license; willingness to transport patients when required.
Physical ability to lift light loads, use of fingers, walk, stand, and operate a vehicle.
