Social Worker (Community Living Center/PACT/Specialty Clinics)

Responsibilities and duties for the Social Worker in EKHCS Community Living Centers, including medical social work for Veterans assigned to PACT and/or specialty teams, and coordination with VA or community service providers.
Responsibilities

The Social Worker provides a wide range of professional services to veterans enrolled in EKHCS Community Living Centers, including high-quality case management, clinical treatment, advocacy, supportive services for Veterans and/or their family members/significant others, and coordination of linkages to VA or community service providers.
Screening/Assessment: assist in the screening process for Veterans applying for Community Living Center (CLC) placement; develop assessments in collaboration with the interdisciplinary treatment team, the Veteran, and family members/significant others.
Treatment Planning/Goal Setting: contribute to treatment plans and set achievable goals with the Veteran and family members/caregiver.
Complete the Minimum Data Set at prescribed transitions (admission, periodic review, significant change, or falls) per CLC standards.
Referral to service providers: act as subject matter expert on VA and/or community resources; collaborate with other service providers to reassess needs.
Educate Veterans and family members/caregivers on available services and assist with referrals based on preferences.
Resource development: use and enhance the VA and community social service program resource files to benefit Veterans.
Advocacy and crisis management: perform rapid assessments and develop crisis management plans; access multiple resource directories for crises.
Discharge/Aftercare planning: assist with discharge/aftercare options and planning.
Administrative responsibilities: enter all Veteran/family contacts in the electronic record using appropriate formats and templates; maintain positive working relationships with VA staff, volunteers, consumers, and stakeholders.
Total rewards and schedule: typical schedule is Monday–Friday, 7:30 am to 4:00 pm or 8:00 am to 4:30 pm (CST); benefits include applicable leave, pay, and federal insurance programs.
Qualifications

Basic Requirements
United States Citizenship: be a U.S. citizen (non-citizens may be appointed when qualified citizens cannot be recruited, as per VA policy).
Education: Master’s degree in Social Work from a school fully accredited by the Council on Social Work Education (CSWE).
Licensure: Must be licensed or certified by a state to independently practice social work at the master’s degree level (GS-0185). See state requirements at VA resources.
Exception: VHA may waive licensure/certification requirements for those otherwise qualified pending completion of prerequisites (applicable at GS-9; for GS-11 and above, licensure/certification is required).
Physical Requirements: See VA Directive and Handbook 5019; English language proficiency: candidate must be proficient in spoken and written English.
Grade Determinations: For GS-11, licensure is required; minimum one year of post-MSW experience at GS-9 level plus licensure/certification; knowledge, skills, and abilities include knowledge of community resources, psychosocial assessment, medical/mental health knowledge, treatment modalities, and ability to provide consultation to staff and students.
Duties

Administrative responsibilities and adherence to facility policies; participate in continuous improvement and communication with teams.
