Role Purpose
Provide support and guidance to all participants that have been registered onto the project.
To develop ongoing action plans with participants which will be reviewed on a regular basis.
To provide one-to-one support and group activities to aid the development of a participant in their recovery.
Work alongside the team structure in delivering services and outcomes for the scheme.
To achieve project targets and outcomes set by Welsh Government.
To support the team leader and referral agents in raising awareness of the service.
To work in partnership with other practitioners to deliver effective interventions and support for people with Mental Health and/or substance misuse.
Provide support and supervision to volunteers.

Gweithio ochr yn ochr â strwythur y tîm wrth ddarparu gwasanaethau a chanlyniadau ar gyfer y cynllun.
Cyrraedd targedau a chanlyniadau'r prosiect a osodwyd gan Lywodraeth Cymru.
Cefnogi arweinydd y tîm a'r asiantau cyfeirio wrth godi ymwybyddiaeth o'r gwasanaeth.
Gweithio mewn partneriaeth ag ymarferwyr eraill i ddarparu ymyriadau effeithiol a chymorth i bobl â Iechyd Meddwl ac
eu camddefnydd sylweddau.
