- Education/Discipline: Physician (MD)

- Experience: Minimum 2 years required
- Licensure: Active California medical license (required, not willing to license)

- BLS (required)
- DEA (required)
- Board Certification: Not required, but preferred (BE accepted)Clinical Information

- Average Daily Census: 18 patients (21+ preferred)
- Procedures: Standard outpatient; further details provided during interview

- Requirement: Providers must attend all days of orientation to be presented
