Position Highlights

• 100% outpatient primary care (no nights, no weekends)

• Average patients per day

• Smaller panel size ( patients)

• Extended visit times for deeper patient care

• Dedicated care team support (RNs, MAs, care coordinators, social services)

• Focus on preventative care and chronic disease management

• Spanish-speaking patient population fluency required

Patient Scope

• Primarily adult and geriatric patients (typically 65+)

• Medically complex population with multiple chronic conditions

  • Common conditions include: diabetes, hypertension, COPD, CHF, and behavioral health components

• High-touch, relationship-based care model

• Emphasis on longitudinal care and improved outcomes, not volume-driven visits

• Opportunity to serve diverse, underserved, and multicultural communities

Requirements

• Board Certified or Board Eligible (Family Medicine or Internal Medicine)

• Active or eligible Texas medical license

• Spanish fluency required
