- Comprehensive Care: Conduct in-person assessments and coordinate individual care plans.
- Clinical Oversight: Manage admissions, discharges, and ongoing care while guiding the team.
- Integrated Role: Provide primary care, collaborate on discharge planning, and cover rotating call.
- Qualifications: Board Certified (Internal Medicine/Family Practice, Geriatrics preferred), active license, CDS, DEA, valid driver's license, and current vehicle insurance.
