Dates of Coverage: ongoing as needed Practice Setting: Urgent Care clinic that offers Occupational Health Documentation System/EMR: Anthem Will APP be Working Autonomously: Yes Supervising Physician Present During Assignment: Yes, supervising physician will be available via phone Procedures: Episodic care, injury treatment Patient Age Demographic: Adolescent, Child, Adult, Geriatric

Facility will reimburse for the following costs : Mileage billed at the IRS Standard Rate - Local providers ONLY

Submission Requirements MUST NOTATE ALL DURING NAME CLEARS

• Board Certified (ANCC, AANP) - REQUIRED
• Active VA license - REQUIRED
• BLS - REQUIRED
• Provider Must be Local - REQUIRED
