Pay Rate: 82,000 (Commensurate with experience).

Schedule: Monday through Friday 8:30am – 4:30pm.

Position Purpose
Responsible for the management of the Restorative, Fall management and Infection Control Programs and coordination of care for our residents.

Supervisory Function
Restorative CNAs.

Resident Rights

Safety and Sanitation

Remains alert at all times, assuring that SSL and its residents are safe and secure at all times.

Actively participates in the SSL Safety Program by direct service on the Safety Committee, or by observing and reporting unsafe conditions to supervisor and by performing all assigned duties with proper body mechanics.

Attends In‑Service sessions as required by facility and supervisor.

Remains task oriented while on the job, with no allowance for horseplay or otherwise distracting oneself or others from the important task(s) of caring for residents.

Primary Job Duties

Implements and evaluates restorative programs assuring they are appropriate for each individual resident.

Instructs, guides, and takes responsibility for the restorative, fall and infection control program.

Create and maintain resident treatment schedule for restorative aides, and delegates specific tasks to all Restorative personnel as warranted.

Continuous assessment and evaluation of the restorative aides’ job performance in addition to the restorative and fall prevention program.

Collaborates with central supply clerk with the ordering, distribution, charges and maintenance of needed equipment and supplies for the adequate implementation of the restorative and fall prevention program.

Completes initial and ongoing assessments of all residents for functional ADL ability, continence status and Range of Motion on a minimum of quarterly basis and upon a change in the resident’s condition or level of performance.

Completes the corresponding MDS sections, assessments and Rap Summaries according to OBRA guidelines.

Involves the resident family and other health team members in the care plan process and informs them of the goals that need to be attained.

Communicates with resident/POA/families regarding resident care via phone, email or in‑person in scheduled intervals or as needed.

Manages and coordinates the mental health program with items such as weekly rounds with MD/NP, medication adjustments, proper documentation and more.

Responsible for all aspects of the respiratory program including, but not limited to fit testing, records management, program monitoring.

Serves as a resource in all restorative, fall and infection prevention matters.

Performs and coordinates the process of surveillance to identify the appropriate use of antibiotics.

Attends the Care Conference prepared and offers suggestions for improved resident care and independence.

Thoroughly explains to residents, family and care team restorative, fall and infection prevention rationale and institutes protocols and monitors results of care delivered.

Attends Utilization/Medicare Meetings and collaborates with the Therapy Department regarding resident progress or the need for services.

Responsible for the provision of the restorative, fall and infection control program to residents with updates to the clinical team.

Coordinates and manages vaccination clinics and the upkeep of the resident immunizations status as required or needed.

Provides orientation to all new employees regarding restorative programs, proper body mechanics, appropriate transfer methods, fall prevention and infection control program.

Performs ongoing in‑service education to staff in all aspects of the restorative and fall prevention programs.

Maintains and manages resident participation log for restorative programs and communicates this information to the billing department.

Acts as primary resource person to nursing staff re: problems related to restorative and fall programs.

With the Director of Nursing implements personnel and expense budget for the programs.

Maintains communication with the Director of Nursing regarding programs and staff performance. Suggests methods for improvement and implementation for new programs.

Is pro‑active in identifying residents that are moderate to high risk for falls, contractures and utilizing preventative measures and proactively in maintaining a restraint‑free environment.

Monitors to ensure that all documentation systems are being completed according to the organization’s policies and procedures.

Participates in Routine quality assurance performance improvement (Q.A.P.I) projects monitoring the implementation and effectiveness of interventions for the restorative and fall prevention programs.

Collaborates with the Dietician and Nursing department in obtaining accurate resident weights.

Any other duties as assigned by the Director of Nursing.

Job Qualifications

Practical Nurse License in good standing.

Minimum 2 years’ experience in Restorative nursing and/or Infection Prevention.

Restorative Nursing Certificate within 90 days of hire.

Infection Preventionist Certificate within 90 days of hire.

Working Conditions

May require shift changes or other non‑standard work cycle that may interfere with off‑duty time.

Potential for mucous membrane or skin contact with blood, body fluids or tissues, or a potential for spills. Potential blood‑borne pathogen exposure.

Subject to possibly hostile and upset residents, family members, etc.
