Requirements

Active New York license
NY Medicaid ID required
NP: Minimum 3,600 hours of experience
PA: Supervising physician assigned
COVID vaccination required
Recent physical (within 12 months)

Clinical Responsibilities

Conduct in-home health assessments for patients
Complete structured evaluations (non-diagnostic, non-treatment role)
Ask standardized assessment questions and document responses
Review patient history and current health status
Submit completed documentation in a timely manner

Visit Structure

90 minutes per visit total:
- 30 minutes: travel prep, patient outreach, chart review
- 30 minutes: in-home assessment
- 30 minutes: documentation and submission

Clinical Environment

Independent, field-based role
No prescribing, treatment, or procedures required
Focus on health assessments and documentation
Laptop provided for documentation
