Support Administrator – Assessor
The Cuyahoga County Board of Developmental Disabilities (Cuyahoga DD) is seeking four (4) full‑time Support Administrators in the assignment "Assessor." SA Assessors will assess and determine funding for DD waiver services and complete assessments for the identified individuals. Training will be provided by the Ohio Department of DD. Assessments must be completed in person in the individual’s home or another chosen location, potentially outside typical business hours. Each assessor is expected to complete about 2 assessments per day.
Responsibilities
Act as the primary point of coordination for the person‑centered planning team and utilize an assess/plan/review/revise process to help teams plan for each individual assigned.
Assist individuals and teams to identify issues, areas of potential risk, and outcomes that are important to and for the individual. Complete and/or coordinate thorough assessments to determine the need for services and supports, revising assessments as needs change and annually.
Facilitate the development of a person‑centered plan, including the determination and direction of needed supports, revising the plan as needed based on individual or team concerns, progress toward meeting outcomes, plan review, MUI prevention plans, or other issues that impact the need for services as described in the plan.
Identify funding sources for supports identified in the Individualized Service Plan (ISP) by utilizing a hierarchy of funding options applicable to each person; plan with teams to access services that are cost efficient and effective.
Establish and approve budgets for services based upon the individual’s assessed needs and preferred ways of meeting those needs; manage fiscal processes related to service authorizations, including use of local and statewide projection tools and prior authorization processes.
Establish and maintain contact with service providers and natural supports to ensure effective coordination of services.
Promote community presence and participation; provide referrals and linkages to community resources to help individuals meet their needs and engage with their communities.
Implement an ongoing system of monitoring continuous plan review to ensure supports are delivered according to each plan, progress is made toward desired outcomes, and individuals are satisfied with services; take action to remedy any immediate concerns regarding health and welfare.
Assist individuals in choosing providers as needed.
Assist with enrollment onto Medicaid waivers as appropriate, completing all related paperwork accurately and in a timely manner; recommend plans for approval to DODD when services are funded through the DODD Medicaid Waivers.
Establish an individual’s eligibility for services of the County Board of DD.
Unusual Working Conditions
Employee will be required to drive extensively both within and beyond Cuyahoga County; reliable personal transportation is essential. Planning meetings will be held in individual/family homes, throughout the community and during evenings and weekends.
Salary
Salary information is covered in a Collective Bargaining Agreement (Union Contract). External hires salary: BA $54,515 and MA $56,150. Current Cuyahoga DD SEIU 1199 employees accept the position as a lateral move with no change in salary.

The ideal SA Assessor candidate will have experience working with a variety of waivers and community settings, and strong assessment skills.
Qualifications
Bachelor’s Degree in Special Education, Social Work, Psychology, or related area.
Experience providing service coordination and/or experience working in a DD program (highly recommended).
The ability to obtain and maintain SSA certification within state statutory timelines.
Hold a valid driver’s license and maintain an insurable driving record according to Cuyahoga DD policy; DL number must be provided during application.
