Responsibilities
This position will be in the Nephrology & Hypertension division, functioning as an NP/PA hospital nocturnist.
The Nephrology & Hypertension NP/PA Hospitalists work on a consulting service to provide 24/7 coverage managing chronic kidney disease (CKD), end‑stage kidney disease (ESKD), acute kidney injury (AKI), hypertension, and acid‑base & electrolyte disturbances for patients throughout the hospital. They collaborate with a multidisciplinary team, including nephrologists, fellows, nurses, and others. The inpatient role requires timely assessments, interventions, interpretation of diagnostic tests, and management of acute and chronic medical conditions. Patient and family education about recovery and disease process is a crucial part of health promotion for the patients.
Primarily focused on Hospital Coverage (days, evenings, nights, weekends).
A minimum of 75% of coverage expected as in‑house overnight hospital coverage.
Work independently to the full scope of practice in compliance with state licensure.
Perform and document in the medical record detailed patient history and physical examinations.
Present patient report to on‑call consultant to develop, collaborate and implement individualized plan of care.
Follow assigned patient population throughout shifts.
Deliver patient education regarding kidney diseases, electrolyte derangements, CKD care and renal replacement therapies, including dialysis modality options.
Consult with surgical teams to promote optimal dialysis access placements for hospitalized patients.
Prescribe appropriate pharmacological treatments and therapeutic procedures, including dialysis and continuous renal replacement therapies, as per accepted evidence‑based guidelines and institutional protocols.
Monitor effectiveness of therapeutic interventions.
Participate in the delivery of Tele‑Health for Advanced Care at Home patients.
Make appropriate referrals as needed.
Assist with ongoing departmental process improvement and policy development.
Actively attend and participate in educational lectures as part of the MD and APP fellowship program, grand rounds, quality improvement and faculty meetings.
Precept, mentor and instruct students, graduates, and NP/PA colleagues.
Qualifications
Nurse Practitioner
Master’s‑prepared graduate of an accredited school of nursing and Nurse Practitioner program. Current FL RN and APRN license and certification as an FNP, ANP or ACNP.
Physician Assistant
Graduate of accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the FL State Board of Medical Practice.
Additional Qualifications
Active BLS certification from American Heart Association or American Red Cross at start date required.
1+ year experience; new graduates may apply.
Inpatient/Hospitalist acute care advanced practice experience preferred.
Prior training and education in nephrology, hemodialysis, peritoneal dialysis, and kidney transplant.
Prior experience providing continuous renal replacement therapy in the ICU setting.
Experience providing care in patient’s homes or via telehealth/remote monitoring.
Strong computer skills in an Electronic Medical Record.
Excellent verbal and written communication skills.
Experience with Microsoft 365 and Teams.
Flexibility to adjust work assignments (outpatient dialysis unit and ambulatory clinic) as needs of the practice evolve.
Strong teamwork ethic with emphasis on collective ownership.
