This is a Career Service (CS) Health Educator position working for the Lee County Health Department. This highly professional role leads responsibilities for linking and re‑engaging Human Immunodeficiency Virus (HIV) positive clients to care across the 7 counties of Area 8.

The incumbent coordinates with HIV/AIDS and STD intervention staff, the data to care team, community partners, and the county jail to identify and re‑engage clients who are not in care.

Programmatic Duties

Use available databases (CARE Ware, STARS, HMS, Provide, the Linkage module, etc.) to re‑engage and link HIV‑positive individuals who are not in medical care, not on antiretroviral medication (ARVs), or have tested positive but have not been engaged into care.

Contact individuals not in care or unaware of treatment options, and work collaboratively with case management support services to ensure continuum of care.

Attend statewide linkage conferences and local case management meetings, and participate in quality assurance and quality improvement initiatives.

Collaborate with local county STD Department to link HIV and STD positive inmates to care upon release from the county jail.

Work closely with Area 8 surveillance officers and case management staff to notify and re‑engage clients who are out of care.

Engage with group committees such as the Regional HIV/AIDS Consortia (RHAC), the Prevention Planning Committee, the Continuum of Care Committee, and the Early Intervention Consultant (EIC) on patient care matters.

Miscellaneous Activities

Attend and participate in linkage conferences, local case management meetings, general staff meetings, and staff development (team/staff meetings, in‑service/continuing education).

Perform related duties as necessary, including public health services during a disaster.

Maintain timely submission of EARs and timesheets.

Knowledge, Skills, and Abilities

Knowledge of social, economic, rehabilitative or health care services, administration, or planning techniques.

Knowledge of principles, practices & procedures.

Knowledge of methods of compiling, organizing, and analyzing data for monitoring and evaluation.

Skill in operating a personal computer and software applications, including Microsoft Publisher, Microsoft Word, and Microsoft Excel.

Skilled in public speaking.

Skilled at listening and asking appropriate questions.

Skilled in written and oral communications.

Skilled at observing and evaluating outcomes of a problem; identify lessons learned or redirect efforts.

Minimum Qualifications

Certified as an HIV/AIDS Counselor and Tester within 6 months of hire.

Must possess a valid driver’s license and have a motor vehicle to carry out duties.
