Job Overview

The Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) is responsible for providing direct resident care and supervising non‑licensed team members in skilled nursing operations and other areas throughout the community as appropriate. The role leads by demonstrating Sunrise Senior Living’s mission to champion quality of life for all seniors, ensuring compliance with federal, state/provincial, and local laws, standards, and Sunrise policies to promote the highest degree of quality care and services for guests/residents.
Key Responsibilities

Clinical Care

Assist the Leadership Team in supervising the nursing team members.

Assist with the development and leadership of the Designated Nursing Assistants (DNA)/Certified Nursing Assistants (CNA) following the Resident‑Centered Care model.

Assist with providing clinical care, including urinary catheterization, tube feedings, dressing changes, Accu‑checks, medication management/administration, admission, and transfer and discharge of guests/residents.

Complete timely and accurate documentation of guests/resident’s charts.

Complete Incident Reports and report to the Director of Nursing Services (DNS).

Review, update, and notate in the electronic health record to document pertinent resident and community information.

Conduct effective rounds at least twice a day and with physicians as assigned by DNS.

Research and document all incidents of abuse, neglect, and misappropriation of funds as assigned by DNS, Executive Director (ED), or Skilled Nursing Administrator (SNA).

Participate in the Clinical IDT meeting and in the Daily Prospective Payment System (PPS) meetings to obtain information on admissions and discharges for the day/week.

Assist in monitoring medication records and administration.

Assist in receiving lab, x‑ray, and other test results and communicate appropriately.

Quality Assurance and Regulatory Compliance

Demonstrate excellent quality care through the Quality Services Review (QSR) process.

Show knowledge of skilled nursing transformation, Resident‑Centered Care Model, and Sunrise and Federal Quality Indicators.

Actively participate in Sunrise Quality Indicator Meetings.

Assist with development of plans to exceed Quality Indicator thresholds.

Demonstrate knowledge of federal, state/provincial, and local nursing regulations, resident rights, RAI process, and participate in surveys.

Restorative

Follow the documentation guidelines of the Restorative process.

Train and monitor DNAs/CNAs in the Restorative program.

Monitor documentation and resident condition changes.

Refer guests/residents to restorative nursing as indicated.

Clinical Systems

Demonstrate Sunrise practices for skin, nutrition, behavior, falls, and bowel/bladder care.

Assist in implementation, training, and monitoring of clinical system documentation.

Consult with physicians as appropriate.

Report concerns or solutions to the DNS.

Medication Management

Transcribe physician orders accurately into the MAR/TAR.

Demonstrate knowledge of the Six Rights of Medication Pass.

Perform medication administration following Sunrise and nursing standards.

Maintain current photo with dates on the MAR/TAR.

Complete Med Pass audit quarterly and MAR/TAR audit monthly.

Monitor medication room, carts, and treatment carts for cleanliness and availability.

Guest/Resident, Family, and Visitor Relations

Respond to requests promptly and warmly.

Communicate with families about medical care and incorporate requests into nursing standards.

Maintain confidentiality of all resident information.

Review, notate, and initial the Daily Log to document resident changes.

Risk Management and General Safety

Partner with the community team to meet occupational health and safety regulations.

Practice safety procedures, including PPE, fire extinguishers, SDS, and lockout–tagout.

Infection Control

Follow infection control practices.

Provide required training.

Handle exposure to blood, bodily fluids, infectious waste, sharps, and hazardous items correctly.

Report occupational exposures immediately.

Store oxygen tanks safely and monitor oxygen levels.

Supervision

Assist with staffing and scheduling to meet resident needs.

Provide team member evaluation, coaching, and counseling.

Lead team members during orientation, trainings, and evaluations.

Training and Team Success

Participate as a team member and work toward team goals.

Demonstrate Daily Credo in interactions.

Serve residents through Principles of Service.

Contribute to engagement programs and surveys.

Attend regular meetings including Town Hall and departmental meetings.

Maintain required training and regulatory certifications.

Perform other duties as assigned.

Experience and Qualifications

Graduate of an approved college/school of nursing.

Current state/provincial license as a Practical Nurse/Vocational Nurse.

Minimum one year of experience in long‑term care, assisted living, skilled nursing, hospital, or full‑service operations.

Current CPR certification with ongoing training required by Sunrise and state/provincial regulations.

Knowledge of nursing practices and federal, state/provincial, and local regulations.

Understanding of Resident Assessment Instrument (RAI) and Care Plan Process.

Proficiency in computer skills, Microsoft Office, and Sunrise applications.

Availability for weekends, evenings, and flexible hours.

Drivers of Sunrise vehicles must sign the Driver job description and understand safety and regulatory duties as applicable.

Pre‑Employment Requirements

Employment with Sunrise is contingent upon completing and passing a drug test (excluding marijuana), a Tuberculosis Test, a physical evaluation, and a background check where required. Covid‑19 and Influenza vaccination may be required per applicable laws and authorities.
