Hospice Licensed Practical Nurse
Job Type: Full-time, Days
Location: Kitsap County, WA
Schedule: Monday – Friday, 8:00am to 5:00pm
Compensation: $36 - $42.50/hour + Benefits

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team. This role provides direct patient care in the home, skilled nursing facilities, and other care settings, ensuring patients and families receive high-quality, individualized support during end-of-life care.

Primary Responsibilities
Provide skilled nursing care to hospice patients in accordance with physician orders, hospice policies, and regulatory standards
Administer medications, monitor patient conditions, and document care accurately
Educate patients and families on care plans, symptom management, and comfort measures
Collaborate with interdisciplinary hospice team members, including RNs, social workers, and hospice aides, to support patient care goals
Respond promptly to patient and family needs, including symptom management and urgent care situations
Maintain compliance with state regulations, company policies, and Medicare guidelines
Participate in weekend on-call rotation
Requirements
Current Licensed Practical Nurse (LPN) license in the state of Washington
Active BLS certification
Previous hospice, home health, or palliative care experience strongly preferred
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal skills
Ability to work independently and manage evening patient caseloads
Valid driver’s license and reliable transportation
