Home Health Occupational Therapist – Providence Medford Home Health
Location:
Serving Jackson and Josephine Counties, Oregon
Schedule:
Part Time, 24 hour/week (Day shift)
Hiring Bonus:
$5,000 for eligible external candidates meeting required qualifications and payment conditions.
Responsibilities
Manages and provides skilled occupational therapy to patients and clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician‑directed plan of treatment, taking into account individual patient special or age‑related needs. Offers independence and flexibility while caring for patients in their most comfortable environment.
Required Qualifications
Bachelor’s, Master’s, or Ph.D. from an accredited Occupational Therapy program.
Within 30 days of hire: National Provider BLS – American Heart Association.
Upon hire: Oregon Occupational Therapist License.
Pay & Benefits
Pay range: $43.64 – $67.74 per hour. Additional compensation may be available for shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
