Job Details
The Community Health Care Worker will screen all patients for Social Drivers of Health, including food insecurity, housing, transportation, and utility insecurity. The Worker will build an external community‑based resource network and connect patients to appropriate resources. Results will be documented and internal referrals made as necessary.

Overview
Please note that this is a grant‑funded role, due to expire at the end of March 2027.

Duties and Responsibilities

Conduct community‑based assessments and interviews to identify and prioritize patient social services needs and objectives.

Screen patients for social drivers of health.

Build relationships with community‑based organizations that offer resources and services.

Connect patients with community‑based organizations.

Follow up to confirm successful connections.

Work with the care coordination team to provide additional resources as needed.

Track assessments and outcomes in the EMR.

Establish and maintain positive relationships with patients, visitors, and employees.

Contribute to and consistently apply Catholic Health policies, procedures, and benefits to all customers and/or employees without discrimination.

Ensure HIPAA compliance and adherence to Catholic Health security policies and procedures.

Other duties as assigned.

Position Requirements and Qualifications

High School Diploma or equivalent.

Community Health Worker Certificate, or to be obtained within the first 6 months of employment.

One year of experience in community work preferred.

Skills

Quick learner and self‑starter.

Works well independently and within a team.

Some community engagement experience, a plus.

Familiarity with data entry and tracking, basic computer skills.

Excellent verbal and written skills; presentation skills a plus.

Experience working and/or engaging in priority neighborhoods.

Strong interpersonal skills, ability to build trust and maintain relationships with community partners and patients.

Strong organizational skills with good follow‑through to complete assignments within specified timeframes.

Bilingual preferred.

Ability to work collaboratively.

Ability to work with families who face exceptional challenges.

Salary Range
USD $26.00 - $27.00 per hour.
