We are seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our post-acute care team in New London, Connecticut. Providers in this role typically see 15-20 patients per day while delivering high-quality, patient-centered care across post-acute settings.

Estimated annual income: $134,000 - $158,000

Deliver high-quality, safe, and cost-effective care in post-acute settings
Provide inpatient rounding in:
Skilled Nursing Facilities (SNF)
Inpatient Rehabilitation Facilities (IRF)
Long-Term Care Hospitals (LTACH)
Collaborate with interdisciplinary care teams in accordance with CMS guidelines
Coordinate care transitions with hospice, palliative care, and home health agencies
Ensure timely documentation and billing in compliance with Medicare guidelines using gEHRimed EMR
Board Certified: FNP, AGACNP, ACNP, AGPCNP, or PA-C
Licensed (or eligible for licensure) in Connecticut
Experience in post-acute or inpatient settings preferred
Willing to train experienced candidates transitioning into post-acute care
Strong communication skills and collaborative mindset
