How You'll Help Transform Healthcare:
As part of the healthcare team, the RN Admission/Discharge assumes primary accountability for nursing coordination and provision of individualized care for patients of all ages. Utilizes delegation and critical thinking skills to provide quality service through a team‑oriented, collaborative approach to patient care. Assists in facilitation of the admission, transfer, and discharge processes.

Responsibilities:

Assesses, plans, implements and evaluates patient care to achieve optimal patient outcomes for patients of all ages as appropriate to service area.

Determines and performs within clinical, leadership and/or education pathways to support direct/indirect quality patient care service and performance improvement by anticipating and effectively functioning in crisis/emergency situations based on sound judgment, escalation, and use of advanced skills.

Manages resources, establishes prioritizes, and modifies environment to meet patient care needs.

Collaborates with case management and social work to facilitate throughput.

Assists in coordinating the admission, transfer, and discharge process across disciplines and service lines.

Actively contributes to the overall positive patient experience through the use of excellent customer service skills.

Utilizes bed tracking technology and collaborates with leadership and transfer center to facilitate throughput.

Interacts with patients, families, visitors and all members of the health care team to initiate a plan of care. Must be able to work independently. Demonstrates leadership abilities along with mature judgement, initiative and proficiency in nursing skills. Demonstrates positive leadership role‑modeling, critical thinking, and problem resolution skills. Assesses data reflective of the patient’s status. Interprets the appropriate information needed to identify each patient’s requirements relative to his or her age‑specific needs and provides care according to department policies and procedures.

What We Require:

Education:

Graduate of a school of professional nursing of an accredited college. BSN preferred.

Experience:

1 year acute care experience required.

Licensure/Certification:

Currently licensed as a Registered Nurse in Virginia required.

Life Support:

AHA BLS-HCP required.

Other:

Demonstrate excellent oral and written communication skills and customer service skills; demonstrate positive leadership role modeling and problem resolution skills. Knowledgeable of basic computer and phone applications. Must successfully complete orientation and maintain requirements. Position requires lifting and long periods of standing, bending, pushing, pulling, reaching, walking, etc. May be exposed to infectious diseases, biohazard and hazardous materials including regulated wastes, and combative patients.
