SCOPE OF ROLE
Reporting to the Director of Reproductive Health, the Coordinator, Social Services – Reproductive Health provides and creates protocols related to counseling and social services for Signature Health’s Sexual and Reproductive Health services. The role delivers services to patients requesting prenatal care or related sexual and reproductive healthcare.
How You’ll Succeed
Provide guidance and education to individuals, couples, and families regarding prenatal services, postnatal services, sexual assault/trauma, and access to mental‑health and case‑management services.
Provide education and consultative services to Signature Health employees as appropriate.
Assist as needed with reporting related to ADAMHS Board‑funded perinatal services.
For prenatal clinic patients:
Administer the Edinburgh Pre/Postnatal Depression Scale, interpret results, and provide support and referral as appropriate.
Conduct psychosocial assessments.
Make referrals for mental and physical healthcare needs.
Refer to area social service/health resources.
Assist with HCAP (Hospital Care Assurance Program) applications for continuity of care.
Assist with transfer to continuing prenatal care providers.
Comply with all agency policies and procedures.
Communicate/outreach with potential partnering providers to grow a community of referring providers.
Perform other duties as assigned.
Requirements
Bachelor’s degree in social work or a related field.
Valid, unencumbered LSW or LPC license in Ohio.
3–5+ years of experience in a behavioral health role (preferred).
Demonstrated ability to make independent decisions, assess, organize, prioritize, and address multiple projects and problems concurrently.
American Heart Association (AHA) Basic Life Support (BLS) certification (required).
