Compensation
$129,000 – $133,000 per year

Job Summary
Supports Evergreen Treatment Services’ mission by evaluating patients, diagnosing opioid use disorder, prescribing and adjusting medication, and coordinating overall patient care across clinic, mobile, and telehealth settings.

Responsibilities

Work with staff to transform health outcomes and address substance use and homelessness.

Serve as the primary medical care coordinator for assigned patients.

Conduct intake assessments, establish diagnoses, and determine initial medication choices and doses.

Evaluate patient doses and adjust medications as needed, via in‑person or telehealth visits.

Recommend appropriate follow‑up care.

Document care decisions in the electronic health record.

Draw blood for lab testing and provide pre‑ and post‑test counseling when required.

Perform HIV and hepatitis‑C testing as appropriate.

Collaborate with outside medical providers for referrals and follow‑up.

Assess urgent or emergent medical needs and determine whether additional assistance is required.

Write medical orders for take‑home privileges and seek CSAT and DBHR exceptions when necessary.

Future Responsibilities (May Include)

Partner with the Primary Care Provider team to enhance patient care and program efficiency.

Facilitate seamless patient transitions within the program, ensuring continuity of care and communication between providers.

Qualifications

Active Advanced Registered Nurse Practitioner (ARNP) license in Washington or active Physician Assistant (PA‑C) license in Washington.

Credential (e.g., Provider One, etc.) and applicable data waiver for buprenorphine (desired but not required).

Full Schedule II and below prescriptive authority from the State of Washington.

Experience in drug abuse treatment (desired).

Knowledge of opioid use disorder medications, side effects, and harm‑reduction principles.

Commitment to racial justice, equity, and a non‑judgmental care philosophy.

Strong interpersonal, verbal, and written communication skills.

Excellent organizational and prioritization abilities, with the capacity to work independently and under pressure.

Ability to maintain confidentiality and use discretion with sensitive information.

Proficient with Microsoft Office; learn quickly and stay current with technology.

Flexible, collaborative, and detail‑oriented team player.

Physical Requirements

May be exposed to illicit drug residues, fumes, and bloodborne pathogens (training provided).

Requires frequent sitting, standing, walking, kneeling, occasional lifting up to 30 pounds, and use of vision and hearing.

Equipment Used

Computer, photocopier, fax machine, phone, medical and dispensary equipment.

Program vehicle (valid driver’s license required).
